• Humana Medicare Advantage plans
• Humana Medicaid plans

You may still receive care at GGC, but your out-of-pocket costs may increase under those plans.

This change applies only to patients insured under:

• Humana Medicare Advantage
• Humana Medicaid

If you have Original Medicare or a different commercial insurance plan, you are not affected.

This change does NOT affect:

• Original Medicare (red/white/blue card)
• Other Medicare Advantage plans
• Commercial / employer-based insurance
• Medicare Supplements (including Humana Supplements)
• Humana Medicare Advantage PPO plans with in and out of network coverage, like KPPA & KTRS retirees

• If you have surgery, pregnancy care, or another ongoing treatment plan, our care coordination team will reach out to you directly.

You can still receive care at Graves Gilbert Clinic. However, visits and procedures may be billed as out-of-network under your Humana plan, which could increase your costs.
